1. I have been informed and understand that the Office of the Vice Provost for International Affairs or one of its units on behalf of Cornell University (hereinafter "Cornell") is making a recording in which my name, likeness, image, and or/voice may be included.
2. I hereby grant Cornell, and its employees and agents, a non-exclusive license to make, use, transcribe, and publish in whole or in part any recorded footage or still images and photography in which my name, likeness, image, and/or voice may be included (hereinafter "Recordings") whether recorded on or transferred to videotape, film, slides, photographs, audiotape, digital format, or other media now known or hereafter developed. This includes, without limitation, a license to edit, mix, duplicate, use, or reuse Recordings as it may desire without restriction as to changes or alterations.
3. I also grant Cornell a non-exclusive license to distribute, display, broadcast, exhibit, adapt, and market any of said Recordings, either alone or as part of its finished productions, and for commercial or non-commercial purposes as Cornell or its employees and agents may determine. This includes the right to use said recordings, still images, and photography for promotion or publicizing any of these uses. All permissions and releases granted by me herein shall extend and apply to Cornell assigns, contractors, sublicensed distributors, successors, and agents.
4. I hereby waive any and all right that I may have to inspect or approve the finished product or printed matter that may be used in connection therewith.
5. I expressly release Cornell and all persons acting under its permission or authority from any claim or liability arising out of or in any way connected with the above uses and representations, including any and all claims for defamation or copyright infringement.

I am over the age of 18 and have read the above release, and fully understand its contents.
